This website brings together a range of information, resources, tools and evidence to support a shift in focus towards personal outcomes, or what matters to people, in children’s and adult services.

Adopting a personal outcomes approach means working with people to find out what matters to them in their lives. It involves collaboration to work out how best to work towards the outcomes identified. It means acknowledging individual strengths and establishing a shared sense of purpose to which everyone can contribute, including the person, their family, carers and other community resources as well as services.
